    Mental health and learning disability nursing students' perceptions of the usefulness of the objective structured clinical examination to assess their competence in medicine administration

    The aim of this study was to evaluate mental health and learning disability nursing students’ perceptions of the usefulness of the objective structured clinical examination (OSCE) in assessing their administration of medicine competence. Learning disability (n = 24) and mental health (n = 46) students from a single cohort were invited to evaluate their experience of the OSCE. A 10-item survey questionnaire was used, comprising open- and closed-response questions. Twelve (50%) learning disability and 32 (69.6%) mental health nursing students participated. The OSCE was rated highly compared to other theoretical assessments; it was also reported as clinically real and as a motivational learning strategy. However, it did not rate as well as clinical practice. Content analysis of written responses identified four themes: (i) benefits of the OSCE; (ii) suggestions to improve the OSCE; (iii) concern about the lack of clinical reality of the OSCE; and (iv) OSCE-induced stress. The themes, although repeating some of the positive statistical findings, showed that participants were critical of the university setting as a place to conduct clinical assessment, highlighted OSCE-related stress, and questioned the validity of the OSCE as a real-world assessment. The OSCE has an important role in the development of student nurses’ administration of medicine skills. However, it might hinder their performance as a result of the stress of being assessed in a simulated environment

Through a rigorous field study of two world-class pioneering corporations, this paper explores how digital disruption is transforming procurement functions.     Dual account video and web conferencing endpoints have both a personal user account and a shared room account that are registered on the same device. When such an endpoint joins a meeting (e.g., after completing a pairing process) it raises a number of security, etc. challenges. To address these types of challenges, various solutions are presented herein through several techniques. In particular, the techniques may include support for devices utilizing proximity in a reverse fashion (i.e., personal mode endpoints may use ultrasound pairing to detect that the personal user is in proximity to a device and allow them to perform authenticated actions directly using the device’s user interface (UI)). The techniques may further provide a way of pairing personal devices with clients without the need for ultrasound or manual pairing

    This is the published version.These Best Practices — also referred to as the CDP Best Practices -- have been created through the collaboration of working groups pulled from library, museum and archive practitioners. Version 1 was created through funding from the Institute for Museum and Library Services through a grant to the University of Denver and the Colorado Digitization Program in 2003. Version 2 of the guidelines were published by BCR in 2008 and represents a significant update of practices under the leadership of their CDP Digital Imaging Best Practices Working Group. The intent has been to help standardize and share protocols governing the implementation of digital projects. The result of these collaborations is a set of best practice documents that cover issues such as digital imaging, Dublin Core metadata and digital audio. These best practice documents are intended to help with the design and implementation of digitization projects. Because they were collaboratively designed by experts in the field, you can be certain they include the best possible information, in addition to having been field tested and proven in practice. These best practice documents are an ongoing collaborative project, and LYRASIS will add information and new documents as they are developed

    MOOSE MODIFY BED SITES IN RESPONSE TO HIGH TEMPERATURES

    Moose (Alces alces) employ physiological and behavioral mechanisms to enable them to dissipate excess heat when ambient temperature is above the upper critical temperature of their thermoneutral zone. In this note, we describe 2 cases where GPS radio-collared female moose modified summer bed sites as a potential thermoregulatory response to high temperatures. The first case occurred on 18 - 21 July 2011 when ambient temperatures averaged 25 °C (8 °C above the upper critical temperature of moose) and reached 32 °C and 96% relative humidity. Based on field observations of the bed site immediately after use, the moose cleared litter and duff to expose 3 m2 of mineral soil under a closed-canopy balsam fir (Abies balsamea) stand. The moose spent 64% of the time bedded during a 4-day event, with ≤11 individual bedding events in the same bed site. A second case was observed on 5 July 2013 during similar weather conditions (29 °C and 70% relative humidity) when a different moose cleared a bed site and used it continuously for 10 hours

    Industrial strategy and the UK regions: Sectorally narrow and spatially blind

    The UK government's new Industrial Strategy could have a significant impact on the country's regions and localities. However, this has received little attention to date. The analysis presented here examines the existing location of the sectors targeted by the first phase of the Industrial Strategy Challenge Fund and the location of the R&D laboratories likely to be first in line for funding. In focusing on an extremely narrow range of sectors, the Fund is likely to have limited impact on the UK's persistent regional inequalities. The activities eligible for support account for relatively little of manufacturing or the rest of the economy and the basis of this targeting and its potential distributional consequences are spatially blind. As such, it runs the risk of widening regional divides in prosperity

    Development and external validation of a mixed-effects deep learning model to diagnose COVID-19 from CT imaging

    BackgroundThe automatic analysis of medical images has the potential improve diagnostic accuracy while reducing the strain on clinicians. Current methods analyzing 3D-like imaging data, such as computerized tomography imaging, often treat each image slice as individual slices. This may not be able to appropriately model the relationship between slices.MethodsOur proposed method utilizes a mixed-effects model within the deep learning framework to model the relationship between slices. We externally validated this method on a data set taken from a different country and compared our results against other proposed methods. We evaluated the discrimination, calibration, and clinical usefulness of our model using a range of measures. Finally, we carried out a sensitivity analysis to demonstrate our methods robustness to noise and missing data.ResultsIn the external geographic validation set our model showed excellent performance with an AUROC of 0.930 (95%CI: 0.914, 0.947), with a sensitivity and specificity, PPV, and NPV of 0.778 (0.720, 0.828), 0.882 (0.853, 0.908), 0.744 (0.686, 0.797), and 0.900 (0.872, 0.924) at the 0.5 probability cut-off point. Our model also maintained good calibration in the external validation dataset, while other methods showed poor calibration.ConclusionDeep learning can reduce stress on healthcare systems by automatically screening CT imaging for COVID-19. Our method showed improved generalizability in external validation compared to previous published methods. However, deep learning models must be robustly assessed using various performance measures and externally validated in each setting. In addition, best practice guidelines for developing and reporting predictive models are vital for the safe adoption of such models

    Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy
